Lots of attempts getting this to work with a car bulb, then got a small 36 ohm 12V panel bulb in a box of bits, which finally burst into life using an LM358N op amp, not Carlson's TLO72 op amp.
Mine worked without the trim pot from ground to pin 3 - a 1k5 only worked. The bulb is just lit.
I then soldered it to perf board, and it ran at 1kHz spot on, 20Vpp out - so no need to add the other amp pair in the LM358N package just yet.
Added 2nd amp = Hi Z in, 100% feedback, low Z out 1:1
